I do hereby agree with Lighthouse Community Church to attend the GUTTS                                                        Weekend on the dates of __________________________.  I will arrive at 11am on Friday at the
DRC, 1415 E. Lancaster in Fort Worth.

I understand that the cost for GUTTS Weekend is $50 per person ($30 for students) which is due two weeks prior to beginning date. Fees can be paid mailed to Lighthouse: P.O. Box 325, Fort Worth, TX, 76101.

I agree to be responsible for my own behavior, to abide by the rules set forth by the GUTTS and/or Lighthouse Community Church staff, and to prepare myself spiritually for this training opportunity.
